Wedding Questionnaire

Welcome to your wedding questionnaire! This is going to help me make sure that everything on your wedding day runs smoothly, that I'm where I need to be, when I need to be there, helping capture your big day! This questionnaire is fairly extensive so be sure to set aside about 20-30 minutes to complete it.*

Helpful Tips and Reminders:

  1. Please be sure to bring both your Save the Dates and a formal wedding invites for me to photograph.
  2. Allow more time than you think you need to get ready! It's better to be safe than rushed.
  3. Make sure there is natural light (window light) in the room where the bride will get getting ready.
  4. If possible, please try to have all the bridal details together before I get there. This includes your dress, rings, shoes, any special jewelry or tokens, veil, garter, etc.
  5. Keep the room in which you are getting ready as clean as possible (keep trash, bags, snacks, etc. all in one area, so it is not spread across the room).
  6. Take a deep breath. Relax and enjoy the day--you're getting married!

*You cannot save your responses and come back to the form, so please look over the form and make sure you have all the necessary information before you begin.

Wedding Vendor Information
This information is important for me to know so I can best tell your story, and is helpful if your wedding is submitted for publication (magazines or blogs). Please complete whatever information applies to your wedding and put "N/A" in the spaces that don't. Every little detail is important. If your dress was your mom's or your earrings belonged to your grandma, be sure to let me know!

Family Formals
Family formals do not need to take more than 20 minutes; however, this is only possible if we are organized beforehand. Please be sure to include first and last names of each family member.
Please include names so that I can call family members by their first name as we organize the portraits. For example: Paige and Groom, Bride's Family: Kathleen (mom), Albert (step-dad), John (dad), Sindi (step-mom), Barrett (brother), etc.
